Booster Shots
Eligibility
The following New Yorkers are eligible for booster doses:
Bivalent Booster Dose
- New Yorkers ages 12 and older are eligible for a Pfizer-BioNTech bivalent booster dose at least 2 months after their previous booster dose or 2 months after completing their initial primary vaccine series.
- New Yorkers ages 18 and older are eligible for a Moderna bivalent booster dose at least 2 months after their previous booster dose or 2 months after completing their initial primary vaccine series.
Monovalent Booster Dose for Children 5-11 Years
- New Yorkers ages 5-11 years old are eligible for a Pfizer-BioNTech monovalent booster dose at least 5 months after completing their primary series.
Additional Dose (Third Dose) for Moderately to Severely Immunocompromised New Yorkers
- New Yorkers ages 6 months through 4 years old who are moderately – severely immunocompromised and received Moderna COVID-19 vaccine can receive an additional (third dose) of vaccine at least 4 weeks after their second dose
- New Yorkers ages 5 years of age and older old who are moderately – severely immunocompromised and received either Moderna or Pfizer, can receive an additional (third dose) at least 4 weeks after their second dose.
- New Yorkers ages 18 years of age and older who are moderately – severely immunocompromised and received a Janssen dose, can receive an addition (second dose) of a mRNA vaccine at least 4 weeks after their last dose.
Initial Booster Dose (Fourth Dose) for Moderately to Severely Immunocompromised New Yorkers
- Moderately to severely immunocompromised 5 – 11 year-olds who received their Pfizer-BioNTech additional dose (third dose) at least three months ago are eligible for a Pfizer-BioNTech monovalent booster dose (fourth dose).
- Moderately to severely immunocompromised 12 years and older who received their Pfizer-BioNTech additional dose (third dose) at least 2 months ago are eligible for a bivalent booster dose (fourth dose) of an FDA-approved or FDA-authorized COVID-19 vaccine. Persons 12 years and older can receive the Pfizer bivalent COVID-19 booster dose, and persons 18 years and older can receive the Moderna bivalent COVID-19 booster dose.
- Moderately to severely immunocompromised 18 years and older who received their Moderna additional dose (third dose) at least 2 months ago are eligible for a bivalent booster dose (fourth dose) of an FDA-approved or FDA-authorized COVID-19 vaccine.
- Moderately to severely immunocompromised 18 years and older who received an additional dose following a Janssen series (second dose) at least two months ago are eligible for a bivalent booster dose (third dose) of an FDA-approve or FDA-authorized COVID-19 vaccine.
What to bring with you to your appointment:
- COVID vaccine card if getting 2nd dose or Booster
- Driver’s License or Picture ID with Date of Birth on it
- Insurance Card

The COVID-19 vaccine providers in Allegany County include: Alfred Pharmacy, Walgreen’s Pharmacy, Fisher’s Pharmacy, Jones Memorial Hospital and Cuba Memorial Hospital. If interested please call the respective location.
COVID Treatment Hotline Available
New Yorkers who test positive for COVID-19, but don’t have a health care provider, can call a new hotline or submit an online form to be evaluated for treatment. All New Yorkers are eligible regardless of income or health insurance coverage.
Outside of New York City:
Call 888-TREAT-NY (888-873-2869) or complete an evaluation at the NYS COVID-19 ExpressCare Therapeutics Access website.

NYS DOH Update on COVID Call Center Hotlines – Hours of Operation Changes
As part of ‘endemic COVID-19’ planning, three COVID call center hotlines will be transitioning to other state agency call center centers for continued operation. The hotlines are:
- COVID
- Vaccine
- Excelsior Pass Plus
Excelsior Pass Plus Hotline
The Excelsior Pass Plus (EPP) hotline is being transferred to the Department of Labor (DOL) call center, effective Monday, 3/27/23 at the beginning of business.
- Current EPP hotline hours of operation are 9 am – 6 pm and this will be changing to on 3/27/23 to the NEW DOL call center hours of operation which are 8 am – 4 pm.
COVID and Vaccine Hotlines
The COVID and Vaccine Hotlines are being transferred to the Office of Child and Family Services (OCFS) call center, effective, Monday 04/03/23 at the beginning of business.
- Current COVID and Vaccine hotline hours of operation are 9 am – 6 pm and this will be changing on 04/03/23 to the NEW OCFS call center hours of operation which are 8 am – 5 pm.
